Dr. Issachar Devine's journey into interventional pain medicine was a natural evolution of his passion for hands-on problem-solving. As an osteopathic physician, he initially used manual techniques to address pain and dysfunction, which later led him through training in general surgery before ultimately specializing in interventional pain management.
"Pain management is a rapidly evolving field, offering cutting-edge advancements that allow physicians to provide innovative solutions for both common and complex pain conditions," he said. "Chronic pain can be physically and emotionally debilitating, and my greatest reward is seeing the relief and happiness on a patient’s face when their pain is alleviated."
As part of Interventional Spine & Pain Institute, Dr. Devine brings over nine years of experience as an osteopathic physician practicing at multiple facilities across New Jersey, Pennsylvania, and Florida. He also holds a faculty appointment as OMM Lab Facilitator at Orlando College of Osteopathic Medicine (OCOM) in Winter Garden, FL, facilitating hands-on Osteopathic Manipulative Medicine instruction and mentoring students in clinical reasoning, palpatory proficiency, and procedural competency ahead of their practical exams and clinical clerkships.
He also serves as a Physician Contributor to the Broward County Opioid Fatality Review Committee in Fort Lauderdale, FL, a multidisciplinary committee convened under Florida's Opioid Fatality Review framework to examine opioid-related deaths and inform county-level prevention, treatment-access, and harm reduction strategy.
He earned a Doctor of Osteopathic Medicine and a graduate certificate in biomedical sciences from Philadelphia College of Osteopathic Medicine, and a Bachelor of Science in finance and management information systems from La Salle University, both in Philadelphia, PA. He completed a General Surgery internship at Hackensack Meridian Health Palisades Medical Center in New Jersey, followed by a residency in Neuromusculoskeletal Medicine and a fellowship in Interventional Pain and Spine in Fort Lauderdale and Miami, Florida, along with a Harvard Medical School Executive Certification in Artificial Intelligence (AI) in Healthcare.
Dr. Devine specializes in kyphoplasty, basivertebral nerve ablation, and regenerative treatments such as platelet-rich plasma (PRP) therapy and Viadisc. "These innovative treatments offer promising solutions for chronic pain and degenerative conditions," he said. "I am passionate about helping patients with osteoporosis prevent compression fractures and improve long-term spinal health, and I incorporate fluoroscopy- and ultrasound-guided techniques to enhance precision, along with Botox therapy for migraines and spasticity to improve patients’ quality of life."
Having trained in an area where pain and addiction were prevalent, Dr. Devine has developed a balanced, patient-centered approach to pain management. "My goal is not just to manage pain — but to resolve it whenever possible. I view pain management as a spectrum."
He takes a stepwise approach: conservative therapies first (physical and occupational therapy, osteopathic manipulation, manual therapy, topical or over-the-counter medications), then minimally invasive interventions (ultrasound- and fluoroscopy-guided injections, regenerative medicine, radiofrequency ablation), with surgery reserved as a last resort. "I emphasize a multidisciplinary approach, incorporating therapy, psychological support, and wellness strategies to help patients regain a balanced, pain-free life," he said. "I believe trust and communication form the foundation of an effective doctor-patient relationship. Some patients build trust quickly, while others take time, and I make it a priority to create an environment where patients feel heard, respected, and actively involved in their treatment plans."

Outside of work, Dr. Devine is an avid Philadelphia sports fan, with his favorite teams being the Eagles and 76ers. He also enjoys learning about new technologies, playing basketball, singing, and dancing.
